How do you solve a murder with no murderer?

Great article by Dr. Shannon McNamara about how the stories that we tell ourselves change how we learn and process adverse events. “After any patient that I cared for died or had an unexpected outcome, I reviewed the case to learn whatever I could. I noticed that when I worried that what happened was all my fault, all I could feel was fear. The shame I felt kept me from being able to learn anything about the case.” There’s a lot to learn here.
